Crossroads Rehab & Nursing is a 139-bed, skilled nursing facility in Vandalia, Ohio, specializing in long-term care and short-term rehabilitation. Our facility also serves the behavioral health community requiring a more secure environment to ensure safety and personal growth. Crossroads is dedicated to exceeding your healthcare and personal service expectations. We promote quality of life and independence through friendly, professional service, and person centered care. We are committed to making a difference, one resident at a time.

Crossroads Rehab & Nursing has been serving it's local community for over 40 years. In the midst of the ever-evolving world of health care and long-term care, we provide exceptional care to our patients and residents, and have remained successful, and beneficial to the community, for keeping our focused on person-centered care.

Whether the need for you or your loved one is long-term care, short-term rehab, respite care, physical therapy, speech therapy, occupational therapy, wound care, memory care, behavioral care, or hospice services, we seek to exceed expectations and help everyone reach their maximum potential. 

Crossroads has a great presence in the community, both by participating in and sponsoring events with the Vandalia Police Department, Breast Cancer Awareness, or the recent water crisis in Flint, Michigan. We are also very involved with the local Senior Center and youth recreation leagues in the community. 

We are dedicated to reaching out to the community by sponsoring special events such as the Vandalia Police Departments National Night Out in August; The annual Boobie-Que Car Show to kick off Breast Cancer Awareness; Recently banning together to send over 100 gallon containers, and over 100 cases of water to Flint Michigan in order to help their water poisoning efforts; We also participate in activities with our local Senior Center and enjoy being a part of their annual bazaar; not to mention other areas of the community by helping the youth recreation sports teams.

Our Activity department continue an active 7 day week calendar for our residents with special events including live monthly entertainment, Happy Hour events, and religious studies events and services. Our Nursing department is led by an excellent Director of Nursing who has an outstanding reputation in the nursing facility. Finally, our fantastic chefs direct and manage Crossroads and other local facilities, and provide outstanding meals each day for our residents. 

We accept MOST insurances, including Medicaid and Medicare, and want to ensure we help each resident and family member find peace during the difficult times of understanding admission into a long-term care facility.
